How is your stress affecting you?
What triggers your stress? - ie consider what kicks off the stress in your life (some may be obvious but getting help might be beneficial - eg to identify unhelpful thought patterns or responses and provide ways of managing these).  Keeping a log of when you feel good and when you feel stressed and why, might help you identify activities, people, thoughts etc that trigger your stress.

What do you do to relax?

What do you do to have fun? And how often?

How good is your diet?

What exercise do you take?
